General Building Rules:

1. Cleanliness Required: Client must maintain cleanliness of all used areas and return spaces to original condition
2. Restricted Access: Access is limited to rented spaces only - no entry to unauthorized areas of the building
3. No Smoking: Smoking is prohibited throughout the entire facility
4. Noise Levels: Maintain reasonable noise levels that do not disturb other tenants or neighbors
5. Capacity Limits: Do not exceed maximum occupancy limit of 125 for safety and fire code compliance

Supervision: Client must supervise all guests and attendees during rental period

TERMS AND CONDITIONS

Payment: A Non-Refundable Deposit of 50% is due immediately after submitting this form to secure event/booking. Acceptable forms of payments the following:

A 5% fee will be added to the total rental fee for any payments made with a credit card.

Cancellation: An event can be rescheduled but there is no refund for cancellation of any event unless otherwise noted in writing and approved by Randy Smith or PdD Studios Representative.
Liability: Client assumes responsibility for any damage to facility or equipment during rental period
Insurance: Client may be required to provide proof of liability insurance for certain events

AGREEMENT

By submitting this form, the client acknowledges they have read, understood, and agree to comply with all terms, conditions, and facility rules outlined in this agreement.
